Make A Referral
A thorough and accurate referral is vital in laying the foundations for a successful placement within the Chadderton Learning Centre, ensuring we gain the clear understanding needed to tailor support effectively for each student.
All referrals are done via the electronic form below, it is important all elements of the form are completed including the risk assessment information. This provides us with a wider landscape understanding of the support already inplace, and required going forward for the student.
Referrals may be made at any point throughout the year, allowing schools the flexibility to seek additional provision whenever it is required.
Please note: that our referral process operates exclusively through a child’s current educational setting; referrals cannot be made directly by parents or carers.
